Disease Management Membership Plans

Disease Management membership is available to those individuals 18 or older who suffer from certain chronic disease conditions. A physician's release and fitness orientation are required before participation can begin.

Fibromyalgia Aquatics Program • Parkinson's Disease Group Exercise Program • Core Stability Training • Cancer Wellness Program • Joint Wellness Program • Multiple Sclerosis Aquatics Program

Corporate Membership Schedule of Fees

Corporate members and additional member(s) from the same household, who are 15 or older. A minimum of 15 employees must participate to receive these special rates.
